Runner’s knee, also known as patellofemoral pain syndrome, is a common cause of pain around the kneecap. In simple terms, runner’s knee is often linked to the way the kneecap moves within its groove at the front of the knee. The patella sits in a shallow channel and is guided by the surrounding muscles, tendons, and ligaments. When the knee bends and straightens, it should glide smoothly without too much friction. If that movement becomes less efficient, irritation can build up around the joint.

This can happen after an injury, because of muscle imbalance, or because the way the lower limb mechanics has changed. Foot mechanics can also play a role. Collapsed arches, hyperpronation or unsupportive footwear can affect lower limb mechanics during running and create an abnormal loading pattern through the knee. When this happens, the kneecap may be pulled slightly out of its ideal track, which can increase irritation and pain.

Symptoms often include pain when running, pain on stairs, aching after sitting with the knee bent for a long time, or discomfort when squatting or kneeling. The kneecap may even be tender to touch and some people also notice clicking or cracking around the knee. This can be unsettling, but it does not always mean there is anything seriously wrong.
